How to Obtain a Child's Birth Certificate in St. Charles, Missouri
Your child may need a copy of his birth certificate for many reasons, including applying for a passport or even registering for school or sports. If you need to obtain your child’s birth certificate in St. Charles, you can do it in person, online or through the mail. However, Missouri law dictates that you provide proper identification, as only the person listed on the birth certificate, one of the parents listed on the birth certificate or the legal guardian of the child listed on the certificate are permitted to obtain a birth certificate.
In Person
Go to the St. Charles County Department of Community Health and the Environment Vital Records Division (scchealth.org) between 8:30 a.m. and 4:30 p.m. Monday to Friday to obtain a birth certificate for your child 1. The office is closed on major holidays.
Present a clerk with a copy of your photo identification, such as your driver’s license, state or government-issued identification, passport or military badge. The department will accept two alternative forms of identification if you do not have photo ID, provided that the additional documents feature both your full name and the name of the agency who issued the form, such as a W-2 form.
Provide the clerk with the name of the child listed on the certificate, your relation to the child and the date of your child’s birth. Once the clerk locates the birth certificate, you have to pay $15 per copy you request, using cash, cashier’s check, money order or credit card. You can take your child’s certificate with you after completing the payment process.
By Mail
Download a copy of the birth certificate application form from the St. Charles County Department of Community Health and the Environment Vital Records Division’s website (scchealth.org) 1. Fill out your name, your relation to the child listed on the birth certificate, the name of the child on the certificate, the date of birth, the place of birth and the sex of the child. You must also provide your contact information, including your home address.
Sign the application in the presence of a notary public, which you can find at any St. Charles bank, tax office or law office or any other federal office.
Include a copy of your photo identification or alternate forms of identification, your notarized application and a check or money order for $15 per copy of your child’s birth certificate that you are requesting.

Fill out the requested information, which includes your name, address, relation to the child, the child’s name, date of birth, location of birth and sex.
